Incidents during lifting operations show a pattern of repeating events, with the UK Club’s work on Incident Prevention showing that an appalling number of these incidents result in life-changing injuries and tragic loss of life. The fundamental causes are ineffectual leadership such as poor mind-sets, attitudes and associated behaviours, and a lack of focus on wellbeing and care.
This Lessons Learnt video details the tragic death of an engine rating when a heavy tin of paint slipped its lanyard and fell from a height of 10 metres.
Watch the video to see what lessons can be learnt from this experience, and while watching, ask yourself: "Could this happen on my ship?"
